Diabetes Self-Management Education and Support Class

Diabetes Self-Management Education and Support (DSMES) is a small group session for a total 4 sessions in a month that allows for group time with the registered dietitian to address nutritional approaches to managing diabetes, along with group class time with nurse educators to explore lifestyle management when living with diabetes. Topics include but are not limited to: survival skills, meal plan, motivation, goal setting, physical activity, glucose testing and diabetes complications. A referral from your primary health care provider is required and typically you will have one individual appointment prior to joining a group class. Please reach out to the Chronic Care Center, Diabetes Services at 712-794-5901 with questions and help getting a referral if needed.
